If you are living with a long-term health condition, our registered GPs can provide structured, ongoing support to help you manage your condition, monitor your health, and reduce the risk of complications all from the comfort of your home.
All chronic disease management care delivered through 1GP is based on a genuine clinical assessment by a fully registered Australian GP, in accordance with Australian clinical guidelines and the standards of the Medical Board of Australia. Your care plan is individualised to your specific condition, health history, and personal goals.
A telehealth consultation with 1GP is suitable for the ongoing management of a wide range of long-term health conditions. Your GP will conduct a thorough clinical assessment and work with you to develop a structured approach to your care.
Conditions commonly managed through 1GP include:
Type 2 Diabetes Ongoing monitoring and management of Type 2 diabetes, including HbA1c reviews, blood glucose assessment, medication management, lifestyle guidance, and referrals to diabetes educators, dietitians, or endocrinologists where clinically appropriate.
Hypertension (High Blood Pressure) Regular blood pressure monitoring support, medication review and management, cardiovascular risk assessment, and lifestyle advice to support long-term blood pressure control.
High Cholesterol and Cardiovascular Risk Ongoing lipid monitoring, medication management for cholesterol-lowering therapy, and cardiovascular risk reduction support in line with Australian guidelines.
Asthma and Respiratory Conditions Asthma action plan review and management, inhaler technique assessment, symptom monitoring, and referrals to respiratory specialists where clinically indicated.
Thyroid Conditions Ongoing thyroid function monitoring, medication management for conditions such as hypothyroidism and hyperthyroidism, and referrals to endocrinology where specialist input is required.
Arthritis and Musculoskeletal Conditions Ongoing GP-level support for osteoarthritis, rheumatoid arthritis, and other musculoskeletal conditions, including medication management, referrals to rheumatology, and coordination with physiotherapy or allied health services.
Mental Health Conditions Requiring Ongoing GP Support Ongoing GP management of mental health conditions including anxiety and depression, medication review and management, Mental Health Care Plan reviews, and coordination of care with psychology or psychiatry services.
Chronic Kidney Disease Monitoring of kidney function, medication review, blood pressure and fluid management support, dietary guidance, and referrals to nephrology where clinically indicated.

Getting started with chronic disease management through 1GP is straightforward:
Select a convenient appointment time through our online booking system. Appointments are available across a range of time slots to suit your schedule.
Before your consultation, you will be asked to provide details about your condition, current medications, and recent test results if available. This helps your GP prepare a thorough and efficient consultation.
At your scheduled time, your AHPRA-registered GP will connect with you by phone or video. They will review your medical history, current condition status, medications, and any recent test results, and work with you to develop or review your ongoing care plan.
Your 1GP doctor will assess your situation honestly and issue appropriate documentation. If a prior period of incapacity can be clinically supported based on your history and current assessment, they will certify accordingly.

What is a GP Management Plan? A GP Management Plan (GPMP) is a formal care plan that your GP can prepare for patients with a chronic condition. A GPMP outlines your health goals, the actions your GP and other health professionals will take, and what you can do to manage your condition. Eligible patients with a GPMP may also be able to access a Team Care Arrangement (TCA), which can provide Medicare rebates for up to five allied health visits per calendar year. Your 1GP GP can assess your eligibility and prepare these plans during your consultation.
