How to Manage Your Chronic Condition Online with 1GP

Getting started with chronic disease management through 1GP is straightforward:

Your 1GP doctor will assess your situation honestly and issue appropriate documentation. If a prior period of incapacity can be clinically supported based on your history and current assessment, they will certify accordingly.

What Happens During Your Chronic Disease Management Consultation?

When you join your 1GP telehealth appointment for chronic disease management, your AHPRA-registered GP will:

  • Review your current condition, recent symptoms, and any changes since your last consultation
  • Assess your current medications and determine whether any adjustments are clinically appropriate
  • Review recent pathology or imaging results if available and discuss their implications for your management
  • Provide evidence-based advice on lifestyle factors relevant to your condition, including diet, physical activity, and other modifiable risk factors
  • Issue prescriptions, referrals, or pathology requests where clinically appropriate
  • Discuss your health goals and adjust your care approach accordingly
  • Advise on the timing of your next review consultation

What is a GP Management Plan? A GP Management Plan (GPMP) is a formal care plan that your GP can prepare for patients with a chronic condition. A GPMP outlines your health goals, the actions your GP and other health professionals will take, and what you can do to manage your condition. Eligible patients with a GPMP may also be able to access a Team Care Arrangement (TCA), which can provide Medicare rebates for up to five allied health visits per calendar year. Your 1GP GP can assess your eligibility and prepare these plans during your consultation.

All consultations are conducted confidentially. Your personal and health information is handled in accordance with the Australian Privacy Principles and the requirements of the Privacy Act 1988.
